Foreign Doctor → U.S. Doctor: The Complete Roadmap

A step-by-step guide for international medical graduates who want to practice medicine in the United States. Written by Dr. Mohammad Chelehmalzadeh, an emergency physician, for the USMLE123 community. New parts are added as they are checked against the official sources.

Updated October 2026. Requirements change. Always confirm current requirements with the official agency (ECFMG, USMLE, OET) before acting. This is general information, not legal or immigration advice.

Part 1: Where do I start?

I am putting together a step-by-step guide for international medical graduates who want to practice medicine in the United States.

First, understand something important:

There is no longer only one route.

The traditional pathway is:

Foreign medical school → USMLE → ECFMG Certification → U.S. clinical experience → ERAS → Match → U.S. residency → state license → practice

But experienced physicians should also know that a growing number of U.S. states have created additional pathways for internationally trained doctors. Some qualifying physicians may eventually obtain a license without repeating an entire U.S. residency.

STEP 1: CHECK YOUR MEDICAL SCHOOL

Before paying for USMLE courses, rotations or applications, make sure your medical school qualifies.

Search the official World Directory of Medical Schools:

https://search.wdoms.org/

Find your school.

Then look at the Sponsor Notes section.

You need the appropriate ECFMG Sponsor Note covering your graduation year.

Simply finding your medical school in the World Directory does not automatically mean you are eligible for ECFMG Certification.

STEP 2: START WITH ECFMG / INTEALTH

Official ECFMG information:

https://www.ecfmg.org/certification/

MyIntealth:

https://www.myintealth.org/

You will establish your identity and begin the ECFMG certification process.

Your medical education credentials ultimately need to be verified.

Do this early. Credential verification can take time.

WHO SHOULD FOLLOW THIS SERIES?

This applies to:

International medical students who want U.S. residency

Recent foreign medical graduates

Older graduates

Practicing physicians outside the United States

Foreign-trained specialists

Doctors who previously applied and did not Match

Doctors who need a U.S. visa

Doctors wondering whether they really need to repeat residency

Part 2: How to become ECFMG certified

PART 2: HOW TO BECOME ECFMG CERTIFIED

ECFMG Certification is one of the main gateways for international medical graduates entering U.S. residency. Here it is in plain English.

1. CONFIRM YOUR MEDICAL SCHOOL

World Directory: https://search.wdoms.org/

Check the Sponsor Notes for your graduation year.

2. OPEN YOUR ECFMG / INTEALTH ACCOUNT AND APPLY

https://www.ecfmg.org/

https://www.myintealth.org/

Your medical credentials get verified. Start early, it takes time.

3. PASS USMLE STEP 1

Step 1 is Pass/Fail.

https://www.usmle.org/

4. PASS USMLE STEP 2 CK

Step 2 CK gives a numerical score and matters a lot in residency selection.

5. COMPLETE A PATHWAY (this replaced Step 2 CS)

Step 2 CS no longer exists. For 2027 there are six Pathways. They cover your clinical skills and communication skills.

Pathway 1: you hold, or recently held, a license to practice medicine without supervision in another country. If you qualify for Pathway 1, you must use it.

Pathway 2: your medical school gives an OSCE that is required for licensure in your country.

Pathways 3, 4, 5: your school is accredited by a WFME-recognized agency, an NCFMEA-comparable agency, or issues a joint degree with a U.S. LCME school.

Pathway 6: for everyone who does not qualify for 1 to 5. Licensed physicians evaluate your clinical skills with the Mini-CEX.

If you failed Step 2 CS in the past, you must use Pathway 6.

Details: https://www.ecfmg.org/certification-pathways/

6. PASS OET MEDICINE

Every Pathways applicant must get a satisfactory OET Medicine score. No exceptions, regardless of native language or where you studied.

https://oet.com/

7. SUBMIT YOUR PATHWAY APPLICATION

A good OET score alone is not enough. ECFMG must review and approve your Pathway application.
