One company. Three products. Every one of them built by a practicing ER doctor.
TebScribe LLC is a New York limited liability company founded by Dr. Mohammad Chelehmalzadeh, a practicing emergency medicine physician. It builds three products: tebIQ, an AI clinical scribe that turns the patient encounter into a structured note; tebChart, a private health-records app that keeps a patient's own discharge papers and medications organized on their iPhone; and tebDictate, a free tool that turns any phone into a wireless microphone for any computer.
What each product is, and who it's for.
tebIQ
An AI medical scribe that listens to the encounter and produces a structured clinical note — HPI, MDM, assessment and plan — in the specialty's own shape. Built on shift by an emergency physician who needed his charts finished before he went home.

Scan your discharge papers and tebChart organizes the medications, the follow-up visits, and the questions worth asking — stored privately on your iPhone, with no health records on TebScribe servers. The same engineering as the clinical product, pointed at the patient side of the discharge.

TebScribe LLC is a New York limited liability company. It was founded and is led by Dr. Mohammad Chelehmalzadeh, an emergency medicine physician who still works clinical shifts — the products are shaped by the documentation problem he has at 2 a.m., not by a roadmap written away from the bedside. tebIQ, tebChart, and tebDictate are all TebScribe products. Each states its own privacy terms on its own site.

Frequently asked
Are tebIQ, tebChart, and tebDictate the same company?
Yes. All three are products of TebScribe LLC, a New York limited liability company founded by Dr. Mohammad Chelehmalzadeh, a practicing emergency medicine physician. tebIQ is the clinician product, tebChart is the patient product, and tebDictate is a free dictation tool anyone can use.
What is tebIQ?
tebIQ is an AI medical scribe for clinicians. It captures the patient encounter and generates a structured clinical note — history, medical decision making, assessment and plan — shaped for the specialty the clinician actually practices. It runs in a web browser, on iPhone, and on Apple Watch. Encrypted in transit and at rest (AES). Never used for training. Never sold.
What is tebDictate, and is it really free?
tebDictate turns any phone into a wireless microphone for any computer. You open the page on your computer, scan the QR code with your phone, and speak — the text appears on the computer. It is free to use, requires no account and no installation, and the connection is end-to-end encrypted. Speech is recognized on your phone.
Who is TebScribe built for?
tebIQ is for clinicians — physicians, residents, advanced practice providers, and the clinics they work in. tebChart is for patients and families organizing their own records after a hospital or emergency department visit. tebDictate is for anyone, clinical or not, who would rather talk than type.
Where can I get the apps?
tebIQ is available in a web browser at tebiq.com and on the App Store for iPhone and Apple Watch. tebDictate works in any phone browser at tebdictate.com — nothing to install. The tebChart iPhone app has not been released yet; tebchart.com is the place to follow it.
